Root Cause Failure Analysis - RCFA

By performing a Root Cause Failure Analysis (RCFA), we estimate the exact failure mechanism timeline from the eventual failure towards the root cause of the failure initiation. The exact failure mode is obtained by inspecting failed components (bearings, gears, couplings…) and retrieving detailed process and maintenance information.

The end-result is a profound understanding of the actual root cause of the failure and how to avoid this failure mode in the future. This is done by implementing the proper maintenance action or tuning the applicable maintenance/monitoring strategy.

Constructing a Failure Mode Effect and Criticality Analysis (FMECA) for a drivetrain and/or performing a Root Cause Failure Analysis (RCFA) after a sudden failure gains important insights of the failure mechanisms. These are needed in order to act on those failures or act on the potential failures. The latter is obtained by implementing the suitable maintenance strategy and -if applicable- condition monitoring topology.
